1. Dust off all your great work from the past year. That includes any work completed in 2007.
2. Click here to download pdf files of the ENTRY FORM and the WORK FORM.
3. Select the appropriate categories that best describe your work and write the corresponding code on the ENTRY FORM and the WORK FORM. If you cannot find a suitable category, you can create your own. If your write-in category falls under one of our general categories please write in the corresponding letter and “other”. If your category does not fall under one of our general categories just write “other”.
4. Fill out the ENTRY FORM (please type all information) and print it out. You may pay by check or credit card. For credit card payment please fill out the information at the bottom of the ENTRY FORM. For checks please indicate check enclosed on the ENTRY FORM. Make your check payable to Creator Awards, LLC. The entry fee is $55.00 per single entry and $75.00 per campaign. For students the entry fee is $35.00 per single entry and $45.00 per campaign. US dollars only. If you are entering a piece in more than one category you must pay the entry fee for each category.
5. Fill out one work form (please type all information) for each piece you will be submitting and print it out. For a campaign print a copy of the work form for each piece in the campaign. If you are entering a piece in more than one category just attach multiple WORK FORMs to the piece. You do not need to submit multiple copies of the piece. If your work is too big to send, you may send an 8 x 10 photograph or proof.
6. If you would like to be notified when your work is received please click here and fill out the CONFIRMATION FORM. Make sure to check the box on the ENTRY FORM for confirmation requested.
7. Send your work with all the completed forms and payment to the Creator Awards, LLC, P.O. Box 326, Cabin John, MD 20818-0326.
All entries must be received no later than May 20, 2008.
